**Location:** Dallas, TX and surrounding areas

**Job Description:**

We are seeking a dedicated Home Health Physical Therapist Assistant to provide in•home care. This role requires independence and a passion for patient care.

**Key Responsibilities:**

• Provide physical therapy in home settings.
• Collaborate on patient care plans.
• Document progress using HCHB and OASIS.
• Manage 20•25 cases weekly.
• Attend weekly in•person office meetings.
• Adapt to flexible scheduling needs.

**Requirements:**

• Valid PTA License and BLS certification.
• 2+ years of home health experience.
• Proficient in HCHB and OASIS (post•Jan 2023).
• Reside within 30 minutes of the territory.

**Contract Details:**

• 13•week, 40•hour workweek.
• No on•call or overtime.
• Pre•approved time off required.

**Additional Information:**

• 2•3 days of orientation.
• Flexible scrub color.
• PPE and parking details provided.
