**Location:** Dallas•Fort Worth, TX

**Job Description:**

We are seeking a dedicated Home Health Physical Therapist Assistant to provide in•home care. This role requires independence and a passion for patient care.

**Key Responsibilities:**

• Provide physical therapy in home settings.
• Document using Homecare Homebase (HCHB) and Oasis E, with same•day submission.
• Manage 20•25 cases weekly, achieving 26•28 productivity points daily.
• Attend weekly in•person office meetings.
• Be flexible to cover various territories.

**Requirements:**

• Valid PTA License and BLS certification.
• 2+ years of home health experience.
• Proficient in HCHB and Oasis E.
• Available for 40 hours per week with flexible scheduling.

**Additional Information:**

• 13•week contract, potential extension.
• No on•call or overtime.
• Pre•approved time off required.

**Work Environment:**

• Scrub color: Any.
• PPE and parking details provided.
